5 V 5 Team Registration
These are friendly matches officiated by volunteer coaches, parents and/or players. Each week your team will be placed in a 4 team round robin tournament playing three 20 minute games. Stronger teams will move-up in playing brackets each week and weaker teams down. Teams play 5 on the field, no goal keepers, no off-sides, no slide tackling, shin guards required.

Registration Procedure
1. Coach/Manager must complete the player team roster indicating Jersey #, Name, Birth Date and ID# (any previous player card acceptable). If registering two teams from the same club, use just one roster form.
2. All players must complete and sign (parent must sign for players under 18 years of age) the Hold Harmless Agreement and Medical Information Form.
3. These forms must be received and verified BEFORE any player is eligible to participate.
4. Before participating each week, all teams/players must check-in at the check-in table.
Any team using non-registered players or players that fail to check-in will lose their playing privilege immediately and with no warning.
